*Our Adobe Acrobat forms have been programmed for you to fill out from your computer once you have downloaded them. To e-mail your completed form, you must click on File, then Print. Select Acrobat Distiller from your printers list. Name it and click save. A saved copy of the form should now appear. Click on File, Send Mail. An e-mail window will appear with your form as an attachment. Simply address it to firstname.lastname@example.org and click send.
